Form ADV-6-1. List of documents submitted by the policyholder to the Pension Fund of Russia

Form ADV-6-1 was approved by Resolution of the Board of the Pension Fund of the Russian Federation dated September 27, 2019 No. 485p. It is presented in Appendix No. 5, the procedure for filling out is set out in Section II of the Procedure (Appendix No. 8 to the said resolution). The need to send applications to receive SNILS, or in the new ADI-REG, for enterprise employees is not related to the employer’s obligation to provide reports to the Pension Fund, this is a legal requirement on the need to have an individual account for the insured person, written in Article 65 of the Labor Code of the Russian Federation.

What does the document look like?

The new form of the accompanying inventory was approved separately by Resolution of the Board of the Pension Fund of the Russian Federation No. 485p dated September 27, 2019. Until February 2020, another form ADV-6-1 was in force, but now it is not used.

The updated form of the document is called “Inventory of documents submitted by the policyholder to the Pension Fund of the Russian Federation” and is one of the reporting forms regarding the provision of information about personalized registration of insured citizens to the Pension Fund of the Russian Federation.

What it looks like and how to fill it out

Let's consider an example in which the policyholder of Svetloe Future LLC submits documents to the Pension Fund after hiring ten employees who do not have an insurance certificate. In this case, filling ADV-6-1 must be done in the following order.

Step 1. Provide information about the policyholder.

Step 2. Enter information about the number of documents sent to the Pension Fund.

As we stated, the sample indicates that the policyholder sent 10 applications to the Fund.

Step 3. Optional.

This block is filled out only if the company uses electronic document management with the Pension Fund.

IMPORTANT!

Organizations with more than 25 employees are required to submit this and other reporting forms electronically. As for small enterprises, they themselves have the right to decide in what format to provide documents. But the use of an electronic format significantly reduces the time spent by HR officers and accountants.

Step 4. Sign.

At the end, the form is certified by the signatures of the head of the organization and the executor. The date is indicated at the bottom and, if necessary, a stamp is placed.

If a document is drawn up in electronic form, it must be certified with an electronic signature.

When to fill it out

The accompanying register is filled out when sending information to the Pension Fund using standardized forms:

  1. Questionnaire ADV-1 (sample of completion and current form in special material) - created when sending individual information of an employee to create an insurance certificate (SNILS or ADI-REG) for the first time.
  2. ADV-2 - if an employee has changed individual data and a replacement (update) of SNILS is necessary.
  3. ADV-3 - sending information to create a duplicate of SNILS if the employee has lost or misplaced it.
  4. SZV-K - when generating information about an employee’s length of service before 2002, it is compiled at the individual request of the Pension Fund of the Russian Federation.
  5. Other documents provided by the policyholder to the Pension Fund of the Russian Federation upon individual requests.

  1. Basic data:
  • Information about the employer who transfers documents to the Pension Fund: TIN, registration number of the organization, checkpoint, abbreviated name of the employer.
  • Other incoming documents that are not included in the list proposed in the form are indicated;
  • The paragraph “number of documents in the package” indicates the number of documents that make up the complete package;
  • The number of specified employees in the SZV-4-2 form is entered in the next paragraph. This number is indicated only in the inventory where this form is available;
  • An internal package number is assigned; this is the serial number of the transferred set of documents entered by the employer. Filled out if documents are accompanied by electronic submission;
  • The next paragraph is filled in by the Pension Fund employee when accepting the package. The employee enters the registration number in the Pension Fund, indicates the incoming serial number and the current year.
  1. The following paragraphs are filled out if the package of documents contains SZV-4-1 or SZV-4-2:
  • The period for which the form data was filled out. It is written only if the reporting period is earlier than 01/01/2002;
  • Registration code for insured persons. This is done in accordance with the code data classifier;
  • The “additional tariff code” is entered from the classifier of the same name. Filled out for those persons on whose account insurance premiums are made at an additional tariff;
  • Information type. Only one of the following items is marked with an “X”:

“initial” – put “X” if information about the employee for the specified period is submitted for the first time, “assignment of pension” is noted in the case of providing information at the request of the employee for the assignment or recalculation of an already assigned pension;

The next item is filled out only if it is necessary to make an adjustment - again necessary about: “corrective” - to make changes to previously submitted information about the employee, in this case the entire volume of data is filled in, and not just the corrected ones, or “cancelling” - filled in for a complete cancellation earlier transmitted data about the employee.

The “territorial conditions” item is filled out in strictly regulated cases according to the classifier and only if the package contains the SZV-4-2 form. Information on the accrued amounts of insurance premiums is entered according to the rules of the SZV-4-1 form, in rubles in total for the entire package of documents, broken down by: additional tariff (if used), insurance and funded pensions.

  1. Filled out for the inventory of forms SZV-1 and SZV-3 with a billing period until 01/01/2002.
  • It is necessary to indicate the reporting period: year, select “X” quarter of the year;
  • Enter information about your total income in rubles and kopecks for the reporting period provided, which will subsequently be taken into account when assigning a pension.

Structure of the accompanying inventory

The standardized register consists of four conditional blocks:

  1. The first block contains information about the policyholder. Here you must indicate the registration number of the institution issued by the Pension Fund, then enter the TIN, KPP and indicate the name. When changing the name or in other exceptional cases, we indicate additional information in the “Note” field.
  2. The second block is a tabular part, which presents the reporting forms named above. Here the policyholder indicates the number of documents sent to the Pension Fund.
  3. The third block is filled out during electronic document management with the OPFR. The block contains the package number assigned by the policyholder’s electronic system upon dispatch, and after receipt, the OPS system assigns a special number to this package.
  4. The fourth block is information about the contractor and the head of the insured organization, their signatures.

Description of the form

Form - accompanying statement to ADV-1 is a form that is conventionally divided into four blocks:

  1. Information about the policyholder - registration number in the Pension Fund of Russia, INN, KPP, short name of the organization, notes in case of name change and some other cases.
  2. A table in which information about the number of documents sent to the Pension Fund is entered.
  3. Block to be filled in when using electronic document management with the Pension Fund of Russia. This includes the number of the package of documents assigned by the policyholder and the registration number of the package with the Pension Fund.
  4. Information about the performer, head of the organization, signatures of these persons, date, seal.

By Resolution No. 2p dated January 11, 2017, the Pension Fund Board approved the forms of documents used for registering citizens in the compulsory pension insurance system and instructions for filling them out.

Updated forms:

  • questionnaires of the insured person (form ADV-1);
  • insurance certificate of state pension insurance (form ADI-1);
  • insurance certificate of compulsory pension insurance (form ADI-7);
  • applications for exchange of insurance certificate (form ADV-2);
  • applications for the issuance of a duplicate insurance certificate (form ADV-3);

Form ADV-1

The insured person's questionnaire in form ADV-1 is a document submitted to the territorial body of the Pension Fund of the Russian Federation when registering the insured person in the compulsory pension insurance system.

The document is filled out personally by the insured person, on the basis of an identity document (List of identity documents).

It is allowed to fill out the document by personnel or other services of the organization authorized by the head of the organization.

The correctness of the information specified in the document is certified by the personal signature of the insured person.

If the insured person is unable to personally certify the document due to a long (more than one month) stay on a business trip, long-term illness or other reason, the policyholder (employer) indicates this reason in the document, which is certified by the personal signature and seal of the organization.

Form ADV-2

An application for the exchange of an insurance certificate in form ADV-2 is submitted to the territorial body of the Pension Fund of the Russian Federation in the event of a change in the last name, first name, patronymic, date of birth, place of birth or gender of the insured person.

The document is filled out personally by the insured person.

It is allowed to fill out the document by personnel or other services of the organization authorized by the head of the organization.

Form ADV-3

An application for the issuance of a duplicate insurance certificate in form ADV-3 is submitted to the territorial body of the Pension Fund of the Russian Federation in the event that the insured person loses the insurance certificate or the insurance certificate is unsuitable for use.

Form ADV -6-1

An inventory in form ADV-6-1 is submitted by the policyholder to the Pension Fund of the Russian Federation as part of a bundle of incoming documents submitted starting from 2002, as well as as part of a bundle of corrective individual information for periods before 2002.

Pension Fund registration number: required. The number under which the policyholder is registered as a payer of contributions to the Pension Fund is indicated, indicating the region and district codes according to the classification adopted by the Pension Fund.

Format: XXX-XXX-XXXXXX. Example: 048-011-002356. (Leading zeros must be indicated!)

Taxpayer Identification Number: required. The taxpayer identification number is indicated.

Checkpoint: required to be filled out. The reason code for registration is indicated.

Using Forms

When a certificate is issued for the first time, the following is submitted to the Pension Fund office:

  • questionnaire of the insured person (form ADV-1);
  • inventory of documents (form ADV 6-1).

They are submitted within two weeks after the employment contract is signed with the employee.

You can submit the questionnaire and inventory in one of the following ways:

  • on paper through a legal (authorized) representative or by mail with a list of attachments in the valuable letter;
  • electronic.

The Pension Fund of the Russian Federation issues SNILS within three weeks from the date of submission of information.

Duplicate certificate

You can obtain a duplicate SNILS at the Pension Fund office. For this purpose the following documents are submitted:

  • application for the issuance of a duplicate in form ADV-3;
  • inventory of documents (form ADV 6-1).

Replacement of certificate

To exchange a certificate, you need to submit to the Pension Fund office:

  • application for exchange (form ADV-2);
  • inventory of documents (form ADV-6-1).

Information is provided within two weeks after the application is received from the employee. The certificate is exchanged within a month.

Issuing a certificate to an employee

The pension insurance certificate or its duplicate is issued to the employee within a week from the moment it is received at the Pension Fund office.

The employee must check the information and sign the accompanying sheet in the ADI-5 form.

It must be returned to the Pension Fund office within a month after the new certificate or duplicate is received.
