Employee questionnaire for personal file

This document is a questionnaire that is filled out by the applicant when applying for a job. In it he must indicate information about himself. After reading the questionnaire, the employer will be able to form a certain impression about the candidate and make an appropriate decision. In addition, the information from the questionnaire is used by personnel department employees to compile a personal file and fill out the personal card of the hired employee. Each employer determines the need to fill out such a document himself, since it is not mandatory when hiring, except for registration for state or municipal service (Article of the Labor Code of the Russian Federation).

The questionnaire for the personal file of an organization employee does not have a unified form. Employers can take as a basis the questionnaire approved for government agencies by order of the Government of the Russian Federation dated May 26, 2005 No. 667-r or independently develop a document form. Developing your own version of the questionnaire is more preferable, since the organization will be able to include in it a list of questions that are needed by a specific department (HR department, legal service, etc.).

The document has a clear structure that allows you to quickly evaluate its content. It consists of several sections. Most often, the questionnaire includes the most significant questions of a social nature: place of residence, education, professional experience, information about close relatives, etc.

When drawing up the questions that will be included in the questionnaire, the employer and authorized officials should not forget about some legal aspects. So, in Art. The Labor Code of the Russian Federation states that it is unacceptable to demand from citizens information concerning their political, religious and other beliefs.

The job applicant must fill out all lines of the application form personally. The document should be filled out legibly and accurately. The compliance of the information entered in the questionnaire with the data specified in the provided documents is checked and confirmed by an employee of the enterprise’s personnel service.

Communication by civil servants of information about Internet accounts

The Ministry of Health and Social Development of the Russian Federation (Ministry of Labor of the Russian Federation) has developed and approved Methodological recommendations for filling out certificates of availability of information and accounts of a civil servant on the Internet.

These methodological recommendations must be used by civil servants when filling out the form for information about existing pages on the Internet containing the necessary information.

Methodological recommendations for filling out the form for submitting information about the addresses of sites and (or) pages of sites on the Internet on which public civil servants posted publicly available information, as well as data allowing him to be identified

These methodological recommendations have been developed with the aim of providing methodological assistance when filling out the form for submitting information about the addresses of sites and (or) pages of sites on the Internet information and telecommunications network, on which a state civil servant or municipal employee, a citizen of the Russian Federation, applying for a government position civil service of the Russian Federation or municipal service, publicly available information was posted, as well as data allowing his identification (hereinafter - form, information, Internet, employee, citizen), approved by Decree of the Government of the Russian Federation of December 28, 2016 N 2867- R.

The procedure for filling out the form for submitting information about the addresses of sites and (or) pages of sites on the information and telecommunications network “Internet”

1.1. If an employee or citizen has not posted publicly available information on the Internet, as well as data allowing him to be identified, the form is not filled out.

1.2. In accordance with the provisions of Article 20.2 of the Federal Law of July 27, 2004 N 79-FZ “On the State Civil Service of the Russian Federation” and Article 15.1 of the Federal Law of March 2, 2007 N 25-FZ “On the Municipal Service in the Russian Federation” ( hereinafter referred to as Federal Law No. 79-FZ and Federal Law No. 25-FZ, respectively) the completed form must be submitted by:

  1. citizens applying for positions in the federal state civil service and the state civil service of the constituent entities of the Russian Federation (hereinafter referred to as the civil service);
  2. citizens applying for municipal service positions;
  3. federal government civil servants;
  4. state civil servants of the constituent entities of the Russian Federation;
  5. municipal employees.

1.3. The form can be filled out either printed or handwritten.

1.4. On the title page of the form, in the spaces provided for filling out, indicate:

  4. the specified information was posted on the website and (or) website page during the reporting period specified in subclause 4 of clause 1.4 of these guidelines.

1.7. The concept of publicly available information is established by Part 1 of Article 7 of Federal Law No. 149-FZ of July 27, 2006 “On Information, Information Technologies and Information Protection” (hereinafter referred to as Federal Law No. 149-FZ).

According to the specified provisions of Federal Law N 149-FZ, publicly available information means generally known information and other information to which access is not limited.

At the same time, the absence of restrictions on access to information presupposes the possibility of an unlimited number of persons to freely receive and, at their own discretion, use posted information without the consent and knowledge of an employee or citizen.

In this regard, addresses of email, instant messaging services (for example, ICQ, WhatsApp, Viber, Skype), as well as sites related to the purchase of goods and services, are not indicated when filling out the form.

1.8. The data allowing the identification of an employee or citizen can be a combination or one of the following information: last name and first name, photograph, place of service (work).

1.9. Sites and (or) pages of sites on the Internet to be included in the table include personal pages of social networking sites, as well as blogs, microblogs, and personal sites.

Questionnaire structure

The application form for your personal file, which you can download below, consists of the following sections:

  • Personal information about the candidate (full name, place of residence, age, citizenship, marital status, etc.).
  • Information about education. Here you need to indicate which educational institutions and when the job applicant graduated, the direction of training, specialty and qualifications for the diploma, information about additional education, number and series of the diploma. This section may also include a question about knowledge of foreign languages ​​and the degree of proficiency in them.
  • Information about work experience and professional skills. Here the applicant indicates the positions in which he previously worked, the address and name of the organization, the date of hiring and the date of dismissal. This section is often presented in the form of a table, which makes it easier to perceive the information.
  • Information about close relatives.

  • Other additional information about the applicant for the position.

What is a personal personnel record sheet and why is it needed?

A personal personnel record sheet is an integral part of an employee’s personal file.
This is a “business card” that contains relevant information: biographical and personal data of the employee. A distinctive feature from the questionnaire is that the questionnaire is filled out by the applicant before being hired, and the personal HR registration form is filled out after being hired.

For budgetary organizations, the mandatory application is confirmed by various local regulations, depending on the field of activity. For example, letter N 03-51/64, dated December 20, 2000, “On Methodological Recommendations for Working with Documents in General Education Institutions” indicates that employees of general education institutions must have such paper.
