Who is eligible for early retirement according to the new legislation in 2021?

The increase in the retirement age in Russia affected not only those categories of workers who can retire upon reaching a certain age, but also beneficiaries who receive the right to a long-service pension. Due to the complexity of their profession, medical workers are included in the preferential list of persons entitled to early receipt of pension benefits.

On March 9, 2021, Russian Prime Minister Mikhail Mishustin signed a decree expanding the list of periods included in the length of service giving the right to early retirement.

Thus, periods of training and advanced training will be included in the work experience.

“The main condition is that during this time employees retain their jobs and salaries, and employers make contributions for them to compulsory pension insurance,” the message states.

Changes to pension legislation in 2021 will also continue for some recipients of early pensions due to preferential reasons.

From January 1, 2021, the rules for retirement will be adjusted for them in the same way as the generally established retirement age.

Such adjustments will also affect medical workers who have the right to receive an early pension upon receipt of the required professional experience.

Recently, officials have raised the issue of completely abolishing preferential pensions for medical workers. There is currently an active discussion of this issue. But so far the disputes have led nowhere.

Conditions for early retirement for medical workers in 2021

Not all medical workers can count on early leave. The list of beneficiaries was approved by Resolution No. 781 of October 29, 2002. Those healthcare workers who meet two criteria at once can retire on preferential terms:

  • Their profession is listed in the List of Resolution No. 781.
  • The medical institution meets the standards established by the List.

Being included in the List allows you to retire based on your years of service. After the pension reform, the conditions for obtaining benefits have changed and depend not only on the number of years worked. According to the law, for a well-deserved rest, a health worker must have the following indicators:

  • Work for a sufficient number of years.
    In accordance with Federal Law No. 400-FZ of December 28, 2013, for employees of rural and township medical institutions, the bar is set at 25 years of work in the profession, for others at 30 years. The length of service for some categories is calculated differently, for example, one year is counted as one and a half years.
  • Accumulate a sufficient number of IPC (individual pension coefficient).
    For beneficiaries in 2021 it was set at 18.6 points, in 2021 it became 21.0. The IPC will be gradually increased by 2.4 points each year until it reaches a maximum of 30 points.
  • Work out a set delay.

The innovations do not abolish length of service, but balance it with standard retirement periods. These measures will help maintain the existing difference between beneficiaries and citizens who do not belong to these categories.

The concept of deferment based on length of service

A deferment is a set period of time that increases the time required for beneficiaries to work.

In accordance with established legislative norms, by 2023 the deferment amount will reach the required maximum of 5 years.

  • At the same time, the increase in the deferment value will occur in stages:
    each year the term of early appointment will be postponed by 1 year (except for the first two years - 2021 and 2021) until the final value is set at 5 years (in 2023).

This will increase the amount of service established for today:

  • for health workers of rural organizations and institutions located in urban settlements, from 25 to 30 years;
  • for other categories of beneficiaries from 30 to 35 years.

List of documents for receiving early pension

To apply for an early pension, the territorial Pension Fund (PFR) must prepare a basic list of papers, namely:

  • application of the established form;
  • passport details;
  • work book and other documents confirming work experience;
  • SNILS (green card);

Additional list of documentation includes:

  • paper confirming the average salary for 60 days of continuous work before January 1, 2002;
  • certificate of change of surname;
  • documentation of change of address;
  • certificate confirming the disability group;
  • document confirming the presence of dependents.

Calculation of preferential length of service for doctors

Preferential length of service is calculated depending on the period during which the work was carried out, and which group of beneficiaries the doctor belongs to.

Conditions that change the rules for calculating preferential length of service

  • For doctors working in rural areas or in urban settlements, the life expectancy is 1.3 years for every 12 months.
  • Specialists who are included in the List of professions subject to special consideration of length of service are awarded 1.5 years of service instead of one year worked.
  • For medical institutions of the Russian Federation located abroad, the length of service is calculated 1:1.

The estimated 12 months are calculated if the physician has worked the required amount of time during the specified period. Until November 1, 1999, this indicator did not affect the benefit. Therefore, part-time workers could also benefit from special conditions, but this generalized approach was subsequently abolished. At the moment, only those periods of time that were worked out according to established standards are subject to accounting.

For health workers who have mixed experience, the calculation is made for each individual entry in the work book. They will be able to retire early only if they accumulate the required benefit period in total. When combining several professions, the work time for each of them is summed up and taken into account if the time standard has been worked out.

How to calculate the preferential length of service of medical workers


Preferential length of service is work experience that allows a certain category of workers, according to the legislation of the Russian Federation, to go on vacation earlier than established by general legislative standards. Accrual of preferential pensions to medical workers is possible only after achieving special length of service.

Special length of service starting from 2021 is established depending on the location of work:

  • You must work in a town, village or village for 25 years;
  • 30 years - in the city.

Several special rules when calculating benefits for doctors:

  1. For such categories of physicians as surgeons, resuscitators, forensic experts, ambulance workers, burn center specialists, not a single calculation coefficient is used (12 months = 1 year of experience), but 1.5. This means that a specialist in this category, having worked for 1 year, receives 1.5 years of service.
  2. Medical workers working in rural areas or combining work outside the city and in the city have the right to use a coefficient of 1.3.
  3. There is a pension benefit for medical employees of military units. For them, a coefficient of 1.6 is used.

For employees who do not apply to the special conditions described above, the standard coefficient is 1.


The calculation of special experience is necessarily based on the calculation of medical experience - i.e. years that a person has worked as a medical worker.

Experience includes:

  • Part-time work in the medical field (total);
  • Temporary disability;
  • Maternity leave;
  • Completion of training to improve qualification skills;
  • Probation periods.

The length of service does not include:

  • Residency and internship;
  • Holiday to care for the child;
  • Vacation at your own expense.

Also, upon retirement, a medical worker may be assigned additional payments according to the coefficient, depending on the region of residence.

What professions allow you to count a year as one and a half years?

Resolution No. 781 approved the List of medical positions, the time worked for which is calculated according to the maximum coefficient. The professions listed in it receive 18 months of preferential service for every 12 months worked.

List of medical positions receiving a 1.5 coefficient of experience

The List includes:

  • Surgeons of all directions and profiles.
  • Nurses helping in operating rooms, working in purulent surgery, as well as burn, trauma, oncology, microsurgical, otolaryngological, orthopedic departments.
  • Midwives.
  • Anesthesiologists.
  • Reanimatologists.
  • Nurse anesthetists.
  • Ward nurses in anesthesiology, resuscitation, intensive care.
  • Forensic experts.
  • Mid-level medical personnel who perform autopsies and forensic examinations.
  • Pathologists and other health workers in pathology departments.

The specified professions must be registered in the work book in the same form as in the List.

Only under this condition is the length of service taken into account at a preferential coefficient.

Other privileges

The state provides other privileges to healthcare workers.

Among them:

  • Medical care at the place of work. It is paid for by the employer.
  • Shorter working day. The work week is 24, 30, 33 or 36 hours (depending on the category of the employee).
  • Special meals for personnel of X-ray departments, surgery and intensive care.
  • Additional days to paid leave – from 14 to 35 days (depending on the place of employment).
  • Additional payments and bonuses, the amount of which is established by the administration of the healthcare institution.

Doctors practicing in rural areas are provided with housing.

  • It is paid for by the local budget.
  • After 10 years of service at this place of work, a health worker has the right to live in this house or apartment permanently, even after retirement due to length of service or age.

The list of additional incentives also includes:

  • provision of land for individual housing construction,
  • compensation for housing and communal services expenses,
  • social mortgage.
